Our office is located at 6242 W. Jefferson Blvd., Fort Wayne, IN 46804, (260) 244-3665. We’re right next to the Pink Poodle Pet Spa in the Covington Plaza shopping center. Parking is available outside the office, and public transportation is nearby.

We are in network with most major providers, Medicare, and Medicaid. All major credit cards, except Amex, and HSA plans, are accepted. Please speak with one of our helpful staff members for further information.
